Track 1:  Organ Transplantation Surgery

Organ transplantation is a medical procedure in which an organ is removed from one body and placed in the body of a recipient, to replace a damaged or missing organ. The donor and recipient may be at the same location, or organs may be transported from a donor site to another location. Organs or tissues that are transplanted within the same person's body are called autografts. Transplants that are recently performed between two subjects of the same species are called allografts. Allografts can either be from a living or cadaveric source.

Track 2. Heart Transplant Surgery & Artificial Heart

A heart transplant, or a cardiac transplant, is a surgical transplant procedure performed on patients with end-stage heart failure or severe coronary artery disease when other medical or surgical treatments have failed. As the most common procedure is to take a functioning heart from a recently deceased organ donor, and implant it into the patient. An artificial heart is a device that replaces the heart. Artificial hearts are typically used to bridge the time to heart transplantation, or to permanently replace the heart in case heart transplantation is impossible. An artificial is distinct from a ventricular assist device designed to support a failing heart. It is also distinct from a cardiopulmonary bypass machine, which is an external device used to provide the functions of both the heart and lungs and are used only for a few hours at a time, most commonly during cardiac surgery.

Track 3. Liver Transplantation & Bio Artificial Liver

Liver transplantation or hepatic transplantation is the replacement of a diseased liver with the healthy liver from another person. Liver transplantation is a treatment option for end-stage liver disease and acute liver failure, although availability of donor organs is a major limitation. The most common technique is orthotopic transplantation, in which the native liver is removed and replaced by the donor organ in the same anatomic position as the original liver. The surgical procedure is complex, requiring careful harvest of the donor organ and meticulous implantation into the recipient. Liver cells obtained from an animal were used instead of developing a piece of equipment for each function of the liver. The structure and function of the first device resembles that of today's BALs. Animal liver cells are suspended in a solution and a patient's blood is processed by a semi permeable membrane that allows toxins and blood proteins to pass but restricts an immunological response.

Track 4. Tissue Engineering & Regenerative Medicine

Tissue engineering is the use of a combination of cells, engineering and materials methods, and suitable biochemical and physicochemical factors to improve or replace biological tissues. Tissue engineering involves the use of a scaffold for the formation of new viable tissue for a medical purpose. While it was once categorized as a sub-field of biomaterials, having grown in scope and importance it can be considered as a field in its own. While most definitions of tissue engineering cover a broad range of applications, the tissues involved require certain mechanical and structural properties for proper functioning. The term has also been applied to efforts to perform specific biochemical functions using cells within an artificially-created support system. The term regenerative medicine is often used synonymously with tissue engineering, although those involved in regenerative medicine place more emphasis on the use of stem cells or progenitor cells to produce tissues.

Track 5. Kidney Transplantation & Artificial Kidney Dialysis

Kidney transplantation or renal transplantation is the organ transplant of a kidney into a patient with end-stage renal disease. Kidney transplantation is typically classified as deceased-donor or living-donor transplantation depending on the source of the donor organ. Living-donor renal transplants are further characterized as genetically related or non-related transplants, depending on whether a biological relationship exists between the donor and recipient. Hemodialysis is a method for removing waste products such as creatinine and urea, as well as free water from the blood when the kidneys are in kidney failure. The mechanical device used to clean the patients’ blood is called a dialyzer, also known as an artificial kidney. The other name for artificial kidney is also called a dialysis machine. Modern dialysers typically consist of a cylindrical rigid casing enclosing hollow fibers cast or extruded from a polymer or copolymer, which is usually a proprietary formulation.

Track 6. Lungs Transplantation & Artificial Lungs

Lung transplantation or pulmonary transplantation is a surgical procedure in which a patient's diseased lungs are partially or totally replaced by lungs which come from a donor. Donor lungs can be retrieved from a living donor or a deceased donor. A living donor can only donate one lung lobe. With some lung diseases a recipient may only need to receive a single lung. With other lung diseases such as cystic fibrosis it is imperative that a recipient receive two lungs. While lung transplants carry certain associated risks, they can also extend life expectancy and enhance the quality of life for end-stage pulmonary patients. An artificial lung is a prosthetic device that provides oxygenation of blood and removal of carbon dioxide from the blood. The AL is intended to take over some of the functionality of biological lungs. It is different from a heart-lung machine in that it is external and designed to take over the functions of the lungs for long periods of time rather than on a temporary basis.

Pediatric organ donation and organ transplantation can have a significant life-extending benefit to the young recipients of these organs and a high emotional impact on donor and recipient families. Pediatricians, pediatric medical specialists, and pediatric transplant surgeons need to be better acquainted with evolving national strategies that involve organ procurement and organ transplantation to help acquaint families with the benefits and risks of organ donation and transplantation. Efforts of pediatric professionals are needed to shape public policies to provide a system in which procurement, distribution, and cost are fair and equitable to children and adults.

Track 9. Pancreas Transplant Surgery for Diabetes

A pancreas transplant is an organ transplant that involves implanting a healthy pancreas into a person who usually has diabetes. Because the pancreas is a vital organ, performing functions necessary in the digestion process, the recipient's native pancreas is left in place, and the donated pancreas is attached in a different location. In the event of rejection of the new pancreas, which would quickly cause life-threatening diabetes, there would be a significant chance the recipient would not survive very well for long without the native pancreas, however dysfunctional, still in place. The healthy pancreas comes from a donor who has just died or it may be a partial pancreas from a living donor. At present, pancreas transplants are usually performed in persons with insulin-dependent diabetes, who can develop severe complications.

Track 10. Transplant Rejection

Immune system usually protects you from substances that may be harmful, such as germs, poisons, and sometimes, cancer cells. These harmful substances have proteins called antigens coating their surfaces. As soon as these antigens enter the body, the immune system recognizes that they are not from that person's body and that they are "foreign," and attacks them. When a person receives an organ from someone else during transplant surgery, that person's immune system may recognize that it is foreign. This is because the person's immune system detects that the antigens on the cells of the organ are different or not "matched." Mismatched organs, or organs that are not matched closely enough, can trigger a blood transfusion reaction or transplant rejection. To help prevent this reaction, doctors type, or match both the organ donor and the person who is receiving the organ. The more similar the antigens are between the donor and recipient, the less likely that the organ will be rejected.

Track 11. Skin & Tissue Grafting

Skin grafts are often employed after serious injuries when some of the body's skin is damaged. Surgical removal of the damaged skin is followed by skin grafting. The grafting serves two purposes reduce the course of treatment needed, and improve the function and appearance of the area of the body which receives the skin graft. There are two types of skin grafts the more common type is where a thin layer is removed from a healthy part of the body like peeling a potato, or a full thickness skin graft, which involves pinching and cutting skin away from the donor section. Grafting refers to a surgical procedure to move tissue from one site to another on the body, or from another creature, without bringing its own blood supply with it. Instead, a new blood supply grows in after it is placed.

Track 12. Uterus & Ovarian Transplantation

The uterine transplant is the surgical procedure whereby a healthy uterus is transplanted into an organism of which the uterus is absent or diseased. As part of normal mammalian sexual reproduction, a diseased or absent uterus does not allow normal embryonic implantation, effectively rendering the female infertile. This phenomenon is known as absolute uterine factor infertility. Uterine transplant is a potential treatment for this form of infertility. He procedure is to take a part of the ovary and carry out slow freezing before storing it in liquid nitrogen whilst therapy is undertaken. Tissue can then be thawed and implanted near the fallopian, either orthotropic or heterotopic where it starts to produce new eggs, allowing normal conception to take place. A study of 60 procedures concluded that ovarian tissue harvesting appears to be safe.

Track 13. Solid Organ Transplant

The field of solid organ transplantation has seen significant advances in surgical techniques, medical diagnosis, selection process. Despite these advances, however, there remains a significant imbalance between the supply of organs available for transplant and the number of patients registered on transplant waiting lists. Notably, the past decade has shown gradual increases in the number of candidates waiting for a kidney, while the number of transplants performed. The waiting list for heart transplants has been the most rapidly growing list. Fortunately, policies designed to improve procurement, screening, and distribution are helping to make transplantation more efficient and organs more accessible, allowing sicker patients to undergo transplants more quickly.

Track 14. Stem Cell Transplantation

Mesenchymal stem cells are multipotent cells found in multiple human adult tissues including bone marrow, synovial tissues, and adipose tissues. Since they are derived from the mesoderm, they have been shown to differentiate into bone, cartilage, muscle, and adipose tissue. MSCs from embryonic sources have shown promise scientifically while creating significant controversy. As a result, many researchers have focused on adult stem cells, or stem cells isolated from adult humans that can be transplanted into damaged tissue. Because of their multi-potent capabilities, mesenchymal stem cell  lineages have been used successfully in animal models to regenerate articular cartilage and in human models to regenerate bone. Recent research demonstrates that articular cartilage may be able to be repaired via percutaneous introduction of mesenchymal stem cells.

Track 15. Retinal Transplantation & Regeneration

Retinal prostheses for restoration of sight to patients blinded by retinal degeneration are being developed by a number of private companies and research institutions worldwide. The system is meant to partially restore useful vision to people who have lost their photoreceptors due to retinal diseases such as retinitis pigmentosa or age-related macular degeneration. Three types of retinal implants are currently in clinical trials The two most well-studied mechanisms of retinal regeneration are cell-mediated regeneration and cellular transplantation. Regenerative processes may have applications in humans for treating degenerative retinal diseases such as retinitis pigmentosa.

An attempt is made to review the fundamental aspects of blood material interactions at the interface and how such concept may be useful in the development of biomedical devices such as small diameter vascular graft, artificial heart, heart valve and drug delivery systems based on micro/nanoparticles. The issues related to technology development, transfer, possible future changes and necessity of regulatory aspects would also be discussed. There may be bioactive materials that can induce healing even in the absence of collagen with optimum adjustment of porosity with respect to its applications and help in the reconstruction of tissues and organs to an almost indistinguishable level.

Presumed consent legislation produces more donors and in particular more organs per donor. In cadaveric donation, the number of elderly donors is increasing, so the quality of organs available is getting poorer. There will be no substantial rise in transplantation unless there is a major breakthrough in the current programs. There will be an increase in the proportion of living related donors and debate about ethics of transplantation. Adverse publicity and debate may affect cadaveric donation and refusal rates The number of kidneys, livers and other body parts surgeons are harvesting through a controversial approach to organ donation has started to rise rapidly, a trend that is saving the lives of more waiting patients but, some say, risks sacrificing the interests of the donors.

Thus, many biotechnology and medical device companies have begun to concentrate on the development of advanced transplant products for management of organ failure. Genomics based diagnostics dominated the overall market in terms of revenue at 36.4% in 2013 majorly owing to the presence of a relatively larger number of R&D programs. Genomics based personalized medicine segment on the other hand is expected to grow at the fastest CAGR of over 12.0% from 2014 to 2020 due to increasing demand for population based therapeutic solutions and subsequent increase in R&D initiatives. Why Tokyo?

The Japanese health ministry has begun a joint research project with transport authorities in an effort to get a handle on the trade. But the government is likely to find it difficult to stop desperate people who have money from making the short plane hop to China. As Hokamura says: "I was on dialysis for four years.Hokamura negotiated the deal through a Japanese broker in Shenyang that operates under the name of the China International Organ Transplant Center which maintains a professional website with detailed information about their services for donors in English, Japanese, Korean and Russian. The website, provides the following information on the sources of organs: If you send your personal data to this center by e-mail or fax and accept the necessary body examination in Shenyang, China in order to assure a suitable donor, it may take only one month to receive a liver transplantation , the maximum waiting time being two months. As for the kidney transplantation, it may take one week to find a suitable donor, the maximum time being one month. Although the procedure to select a donor is very strict, the transplant operation will be terminated if the doctor discovers that there is something wrong with the donor's organ.” The source of the organs, however, is not specified. 

 Organ Transplants in Japan

Many patients in Japan are bitter about the underdeveloped state of transplant health in a country that has seen less than 50 cases of donated organs since the revised 1997 Organ Transplant Law. “Doctors in Japan are happy with their patients being on dialysis because it is profitable,” claims Hokamura. “They get 5.1 million yen a year to treat people like me.”According to health ministry statistics, there have been a total of 998 kidney transplants in Japan but the bulk of these -- 800 – have been from living persons. The rest are from people who died of brain death or whose heart stopped. A single death can on average now save 10 people, but there are still far too few donors and with 12,000 patients waiting for matching donors, according to the Japan Organ Transplant Network, the government is again preparing to revise the law. Meanwhile, increasing numbers of Japanese are turning to China. Sources say the cost of a kidney transplant in China runs to US$66,500 and a liver up to US$157,000. A Taiwanese broker called Yeson Healthcare Service Network recently told the Japan Times that a heart transplant at the Zhongshan Hospital in Shanghai can be had for as little as US$119,000, a fraction of the US$860,000 such an operation would cost in North America.
